A free GOV.UK service. It tells most government departments at once, including HMRC, DWP, DVLA, the Passport Office and the council. Use the reference number the registrar gives you.

Before you get in touch

You can tell AXA Health by phone, with their online form, or by post. Have the member's name, address, date of birth and membership number if you have it. If there is an executor, they will want their details. They may ask for a copy of the death certificate and grant of probate, and will talk you through it.
